Internet Applications of US Visas

All applicants are required to complete an EVAF DS-156, no hand/typed written application form will be accepted.  All male non-immigrant visa applicants regardless of age must fill out and submit a DS-157.

Note that some applicants may require other forms in addition to the DS-156. All student visa applicants must complete and submit form DS-158 and all males to complete DS-157.

Each EVAF DS-156 form will have a barcode used for scanning the application when you appear at the Embassy for your appointment.  Unacceptable barcodes (those that are not read by our scanner due to poor printer quality and other factors) will not be accepted and applicants will have to exit the Embassy and return with a readable barcode.
