All applicants are required to complete an EVAF DS-156, no hand/typed written application form will be accepted.

Note that some applicants may require other forms in addition to the DS-156. All male non-immigrant visa applicants regardless of age must fill out and submit a DS-157, and all student visa applicants must complete and submit form DS-158.
