Replacing a Lost/Stolen/Mutilated Passport

All applicants whose passport was lost/stolen/mutilated must apply in person.

We work on replacement passports only during regular business hours, except in the case of clear, documented, life-or-death emergencies, such as the death of a close family member in the U.S. Otherwise, no weekend or after-hours assistance is available. Normal processing time is about three to four weeks.

The American Embassy is closed on both U.S. and Fijian holidays.

Emergency passports issued abroad are valid for no more than 12 months (and may be issued with much shorter periods of validity), and must be replaced by a full-validity passport printed in the U.S. You must return the limited validity emergency passport to obtain a full validity (5 or 10 years validity) passport. Processing a replacement passport may be longer than normal, if, for example, you have shown a pattern of multiple losses. You should not make any unchangeable travel plans until we are able to hand you your new passport.

Here's What You'll Need

To replace a lost or stolen passport, you must present evidence of your identity and your citizenship. You will also need a police report, fee, photo and an application form.

• Proof of Your Identity
  

  You may prove your identity with any one of these, if you are recognizable :

• Previous U.S. passport ( mutilated, altered, or damaged passports are not acceptable as proof of identity .)

• Naturalization Certificate

• Certificate of Citizenship

• Current, valid

• Driver's license

• Government ID: city, state or federal

• Military ID: military and dependents

FOR MINORS UNDER THE AGE OF 16: Each minor child must appear in person. Both parents or legal guardians must present evidence of identity when they apply for a minor under the age of 16. Read more information on the identity requirements for minors under the age of 16.

FOR MINORS under the age of 16: 

• Your child MUST appear personally with their parents to establish identity.
• Proof of citizenship and proof of relationship.
• For security reasons, parental consent may be requested.
• If your child does not have identification of his/her own, you need to accompany your child, present identification and co-sign the application.

NOTE: Your Social Security Card does NOT prove your identity.

• Proof of U.S. Citizenship

You may prove U.S. Citizenship with any one of the following :

• Previous U.S. Passport ( mutilated, altered, or damaged passports are not acceptable as evidence of U.S. citizenship .)
• Certified birth certificate issued by the city, county or state
  NOTE: A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar’s signature, and the date the certificate was filed with the registrar's office, which must be within 1 year of your birth
• Consular Report of Birth Abroad or Certification of Birth
• Naturalization Certificate
• Certificate of Citizenship
• A Delayed Birth Certificate filed more than one year after your birth may be acceptable if it has listed the documentation used to create it and it is signed by the attending physician or midwife, or, lists an affidavit signed by the parents, or shows early public records.

NOTE: FOR MINORS UNDER THE AGE OF 16: The citizenship evidence submitted for minors under the age of 16 must list both parents' names.

• Police Report
  To replace a lost/stolen passport, also submit a police report documenting the circumstances surrounding the loss or theft. If the police will not issue a report, they will usually issue a report receipt, or a report number. Please bring this in to us with your application materials in lieu of the actual report.
  
• Affidavit
  Please also submit a detailed affidavit of the circumstances surrounding the loss or theft. Here is the form to use, available for download: DS-64. Please don't sign the affidavit-- you will need to sign it at the Embassy when you apply in person for a new passport.

You will need to use the latest version of free Acrobat software to view and print this form. You can download the software here.

Note that the form you print must be clear and sharp, with black print on white paper. The paper must be 8 1/2 inches by 1l inches. Thermal paper, dye-sublimation paper, special inkjet paper, and other shiny papers are not acceptable.

  Please submit one copy of Form DS-11, available for download (forms are also available for pick-up at the U.S. Embassy Suva)

• Fee
  The fee to replace a lost or stolen passport is $100 for adults and $85 for those age 16 or younger.

We accept only cash (US$ or Fijian dollars), money orders or bank drafts. We do NOT accept personal checks or credit cards.

NOTE:

• Minor Applicants under the age of 16 must meet the requirements imposed by the Law on Passport Applications for Minors.
• Persons replacing Official or Diplomatic passports must submit form DD 1056 or USG orders in lieu of the fee.
